We regret to inform you that we have made the difficult decision to permanently close Bluestone Family Medical Centre, effective Friday 1st July 2022.
Our priority at this difficult time is to ensure the continuity of care of our patients.
With this in mind, following the closure of Bluestone Family Medical Centre, Dr Thadi will be available to continue to see patients at his new location at Q1 Medical at Highpoint (Level 2, Shop 2540/120-200 Rosamond Rd, Maribyrnong VIC 3032).
Your medical records will be safely retained by Dr Thadi (at Q1 Medical) for the required legislative period; during which time your medical records will be available for access, (for example, if you wish for them to be transferred to your new treating GP).
We understand and apologise for the disappointment that this news may cause. We continue to be available to you until the 30th of June 2022 to assist you with the smooth handover of your medical care.
We wish to thank you all for your support, particularly over these past two very difficult years during the Covid-19 pandemic.
If you wish to have access to your records or for them to be transferred to another practice, please email your request to thadi@bluestonefmc.com.au Along with the name and contact details of your new GP/Medical Practice, together with a signed authority to transfer your record.
