CLINIC CLOSURE

We regret to inform you that we have made the difficult decision to permanently close Bluestone Family Medical Centre, effective Friday 1st July 2022.

Our priority at this difficult time is to ensure the continuity of care of our patients.

With this in mind, following the closure of Bluestone Family Medical Centre, Dr Thadi will be available to continue to see patients at his new location at Q1 Medical at Highpoint (Level 2, Shop 2540/120-200 Rosamond Rd, Maribyrnong VIC 3032).

Your medical records will be safely retained by Dr Thadi (at Q1 Medical) for the required legislative period; during which time your medical records will be available for access, (for example, if you wish for them to be transferred to your new treating GP).

We understand and apologise for the disappointment that this news may cause. We continue to be available to you until the 30th of June 2022 to assist you with the smooth handover of your medical care.

We wish to thank you all for your support, particularly over these past two very difficult years during the Covid-19 pandemic.

If you wish to have access to your records or for them to be transferred to another practice, please email your request to thadi@bluestonefmc.com.au Along with the name and contact details of your new GP/Medical Practice, together with a signed authority to transfer your record.

Billing

GENERAL PRACTICE

Bluestone Family Medical Centre is a Mixed Billing Practice.
Bulk-billing is available to the following patient groups who hold a valid Medicare Card:

• Children Under 16 years of age
• People over 65
• Commonwealth Health Care Card and Pension Card holders
• DVA Gold and White Card Holders
• Follow up Appointments for results etc..

A Private Fee applies to all new patients to the clinic who do not fit into the abovecategories. The consultation fee of $80.00 will be charged at the initial consultation.Subsequent follow up visits relating to the initial consultation may be Bulk Billed. If you have not been seen at the clinic for a period of 3 months or may an initial Private consultation Fee may again be payable.

We charge $80 for Level B services (Most common services in General Practice), please enquire at reception or call 03 9397 2800 for a list of fees for other consultation items, procedures, reports, pre employment medicals, ..etc

DIABETIC EDUCATIOR

We bulk bill patients under EPC Care plan to see our Credentialed Diabetic Educator, Lisa Taliana. Therefore no out of pocket expenses apply if you have a chronic disease with Team Care Arrangement plan in place (item 723) and a valid referral, please ensure to bring this paperwork with you.

Otherwise we charge $60 for initial visit to see our CDE while $50 for any subsequent visits.
